The table below explains the fields of the form; the top 3 rows should repeat the data in the Roster, Web edition (click here for Roster). If you do not tell me your name and call I won't know who you are!

Data Entry Details
Roster Call/ID Required, enter your call sign or personal ID from web version of Roster.
My Name Required, enter your name from web version of Roster.
My listed email Enter your email address from web version of Roster if one appears there.
Add my email address Add your email address or change it.
Change my call sign If your call sign has changed, enter new one here.
Change my USPS address and/or phone number Enter the address you want appear in the Roster and the Short Skip mailed to.
Alternate USPS address Seasonal visitors may have Short Skip sent to a second address part of the year instead of the address above.
Month start What month should we START sending to alternate address.
Month stop After what month should we STOP sending to alternate address.
Comments Questions and additional remarks may be entered in this field.
